Report a Title IX Incident
This form may be completed by any student, vendor, visitor, or employee of LMU to report an alleged violation of LMU’s Title IX Policy. The University’s Title IX policy prohibits discrimination based on sex, including but not limited to sexual harassment such as sexual assault, domestic violence, dating violence, and stalking. While as much information as possible is appreciated, you may still submit this form if you do not have all of this information.

File a Formal Complaint

At the time the formal complaint is filed, the Complainant must be participating in, or attempting to participate in, LMU or JFWA’s education program or activity. A formal complaint may be filed by the Complainant by:
• Completing the online form,

Request an Appeal
